EYEMED VISION DISCOUNT

The EyeMed Vision Discount program is available to Humana dental plan members. This program is not an insurance plan, but rather a discount program for eye exams, glasses, contact lenses, laser vision correction, etc. There is no additional cost for this program.

EMPLOYEE ASSISTANCE PROGRAM (EAP)

All City employees and their immediate family members have access to an Employee Assistance Program though the Employee Resource Center (ERC). The ERC provides confidential, professional, counseling services to help resolve personal concerns that can affect your family life, health or work life. The City sponsors this program and there is no cost for the counseling visits. Employees have eight (8) visits per subject; if more visits are needed, the ERC will recommend other resources. This program is completely confidential and the City is never provided names of participants.

 

EAP Incentive Program. If EAP services are first sought through the Employee Resources Center (ERC), and the member is referred for outside treatment, the deductible will be waived.

LONG-TERM DISABILITY INSURANCE

The City sponsors the long-term disability program and there is no cost for employees. Long-term disability insurance is through The Standard. In the event you become disabled and are unable to work, this policy provides 60% of wages after 90 days off. Coverage is effective after the first full month following date of hire, i.e., third month of employment.

SECTION 457 DEFERRED COMPENSATION PROGRAM

This program allows employees to defer a portion of their salary for future supplemental retirement income at their expense through payroll deductions. The amount deferred reduces state/federal income taxes and earnings on these deferrals accumulate tax-free until withdrawn.

 

Enrollment is voluntary and you can enroll and/or change the amount you defer at any time throughout your employment by completing the provider's forms. There are two plans to choose from, Wisconsin Deferred Compensation (WDC) and Nationwide Retirement Solutions. Each plan has its own enrollment requirements and mutual funds to choose from.

WISCONSIN RETIREMENT SYSTEM – STATE PENSION PLAN

The State's Department of Employee Trust Funds (ETF) administers the Wisconsin Retirement System (WRS). Eligible employees are automatically enrolled upon employment. Required contribution rates are set by the ETF, and employee contribution rates are taken pre-tax. An annual Employee Benefit Statement and the quarterly newsletter will be distributed to employees via interoffice mail.

WISCONSIN GROUP LIFE INSURANCE

The State's Department of Employee Trust Funds (ETF) administers the Wisconsin Retirement System (WRS) and the Life Insurance Program. Enrollment is voluntary. Premium rates are based on age, earnings, and number of units purchased. Coverage options: Basic, Additional, Spouse & Dependent; purchasing Basic is a prerequisite for purchasing additional units. The effective date for life insurance for a new employee will be the first day of the month following 30 days from the date of hire.
